I understand that the Registration Fee is not refundable.
I understand that the Supply Fee is due by July 1 and is not refundable after July 1.
I understand that the first tuition is due by August 1.
I understand that tuition is payable August through April, and school begins in September and goes through May.
I understand that my child must have a current immunization record or a letter of exemption on file. I understand that my child cannot be admitted without this form, and it is my responsibility to keep forms current.
I agree to notify the preschool, as a courtesy, if my plans change, and I will not be sending my child to First Baptist Preschool & Kindergarten.
